# Customer intake form questions prompt: free ChatGPT template

Use this when you're building or fixing a booking form, request form or new-customer form. Too few questions and you call every customer back; too many and people give up halfway. AI helps you think it through, but it loves to add fields "just in case," including ones you shouldn't collect at all, like full birth dates or card numbers. This prompt starts from what you actually need to do the job, keeps sensitive data out, and explains every field so you can cut the ones that don't earn their place.

## The prompt

Copy it into ChatGPT (or Claude, Gemini or Copilot) and replace every `[BLANK]` with your own details. It uses the four parts from [Prompt Writing 101](https://learn.ynteractive.com/content/prompt-writing-101-role-context-task-format): Role, Context, Task and Format.

## Fill in the blanks

Your business name and what you do.

Booking request, quote request, new client intake, service request.

**`[MUST KNOW]`**

What you truly can't work without, as you'd ask it on the phone.

**`[NICE TO KNOW]`**

Things that help but can wait for a call, such as how they heard about you.

**`[AFTER SUBMIT]`**

What really happens next, such as "we email a confirmation and call within one business day."

**`[MAX FIELDS]`**

A hard limit. Eight is plenty for most small business forms.

## Example, filled in

**A made-up example** from the free lesson [Find and Use n8n Templates](https://learn.ynteractive.com/content/find-and-use-n8n-templates). Hillside Pet Grooming is the made-up groomer in the Find and Use n8n Templates lesson, where a template logs booking requests to Google Sheets and emails a confirmation that names the pet. Here the owner designs a better booking form for it.

## What a good answer looks like

- "Pet name" as its own field, since the confirmation email in the lesson uses it.
- Service as a dropdown (bath, full groom, nail trim) instead of free text.
- A "Anything we should know about your pet?" box with help text like "nervous with clippers, sore hip."
- "How did you hear about us" in the left-out list, saved for the phone call.

## How to check it before you use it

An AI draft can sound right and still say something you never told it. Use the habit from the [Reply Faster lesson](https://learn.ynteractive.com/content/chatgpt-reply-to-customer-emails): check every promise, name, date and number against what you gave it before anything goes out.

- Check every promise in the thank-you message: "we'll call within one business day" is fine only if someone actually does.
- Make sure field labels match your sheet's column names exactly, or your automation will put data in the wrong place.
- Cut anything you won't use in the first conversation. A field nobody reads is a reason for people to quit the form.
- Read the health and behavior field help text to be sure it asks for what the groomer needs, not a medical history.

Asking the AI to explain every field and list what it left out turns form design into a decision you can review instead of a list you have to trust.
