Quick answer

An intake form is the form a business uses to collect the details it needs from a new client or request. One consistent form replaces scattered emails and phone calls, so every new client starts with the same information.

## Why it matters for a small business

Without an intake form, onboarding happens in pieces: a call here, an email there, a missing phone number found later. An intake form puts the questions in one place and in the same order every time. Staff don't have to remember what to ask, and clients don't have to answer the same question twice.

A good intake form asks only for what you will use. The basics are usually a name, an email, a phone number, a company and the type of service wanted. Everything else can wait for a conversation. Fewer fields means more people finish the form.

Because a form collects personal details, think about where the answers go and who can see them. A sheet or inbox that the whole office can open is fine for a name and an email, but not for sensitive documents.

## In a real lesson: Jotform AI: Build a Client Intake Form from One Prompt

In the [lesson](https://learn.ynteractive.com/content/jotform-ai-client-intake-form), Brightline Accounting, a made-up firm, onboards new clients by email and wants one clean form instead. Jotform AI builds it from one sentence, with name, email, phone, company and service type required.

The same idea appears in the [n8n lesson](https://learn.ynteractive.com/content/n8n-execute-step-execute-workflow-publish), where a new-client form feeds a Webhook, Edit Fields and a Google Sheets row at Maple Street Bookkeeping. In both lessons the form is the front door, and what happens after submission is a separate step.

## Common confusions

### Intake form vs contact form

A contact form says hello. An intake form collects what you need to start work, such as the service wanted.

### Intake form vs engagement letter

An intake form gathers information. An engagement letter is the agreement. Do not treat the form as a contract.

## Tips

- Mark only the fields you truly need as required.
- Add a field for the service, so you can route requests later.
- Test the form with a made-up client before it goes out.
